Membership starts at $125 per month, billed monthly, with a six-month initial term. The floor is deliberately low — not as a discount, but as the mechanism: members who contribute above it fund sponsored memberships for patients experiencing hardship. Every member receives the full physician relationship: direct access, extended visits, coordination, and continuity.
Your physician does not know what any patient pays. All tier placement is handled by staff. Every member receives the same standard of care.

Family Add-On

Full clinical care for children of a full-rate member. Same access, same physician. Transitions to standard membership at age 26.

Sponsored Memberships
Up to 15% of our panel is reserved for sponsored memberships, with a goal of 20%. Contributions above the $125 floor fund this program.
All memberships require monthly autopay. Spouses pay the full membership rate. No limit to the number of children at the family add-on rate, so long as the parent is a full-rate member. For Medicare Part B members, the membership fee covers concierge access only; clinical services are billed to Medicare, and standard Medicare cost-sharing applies.
If the membership rate creates a hardship, please contact our office. Sponsored memberships are offered as capacity allows.
